Below are links to the forms needed for TYE employees to join the BLET. Please download and complete one of the forms below. After downloading, these forms can be filled out and signed electronically. Once completed, please email the completed form to our Secretary Treasurer Darice Gilliam. These can also be printed, filled out and returned.
BLET Membership Application
Use this form if you are currently a SMART-TD member and wish to join the BLET.
BLET Transfer Form
Use this form if you are already a member of another BLET division, are working out of Needles or Barstow and wish to transfer to Division 383.
